- To help you prepare, here is a list of documents you must bring with you when you visit our clinic for your appointment.
- Photocopy of Original passport
- 2 latest 35 mm × 45 mm sized photographs with white background not older than 6 months without obstructing the face and resembling the current appearance.
- Any document with your address in Malaysia with Post Code. University/office address is also fine, For eg. offer letter, admission letter, etc if you don't have any letter, self written letter/email with Malaysia address is also acceptable.
- Certificate once issued is final and no changes can be made once issued thus make sure you verify your certificate before you leave our clinic
- Please note there is no fasting required for this medical test
- It will take approximately 3 hours to complete your medical examination and we will hand over the report immediately
- Kindly note, certificate will be handed over only to the applicant. We will require applicant's signature on the certificate
- We do not do test during pregnancy. Please talk to doctor if you have pregnancy.

- Double check the certificate Certificate once issued will be considered final and no changes can be made after that. We urge applicants to double check the accuracy of the contents on the certificate
- Immediately bring it to the attention of the clinic staff for any changes/ correction if required.
